29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    a. Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 14, 2015, employees were exposed to electrical hazards in that the flexible power cord for the DeWalt circular saw (model DW364) the cord's sheathing/parts which protect the electrical equipment was damaged.     b. Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 14, 2015, the flexible power extension cord to which the DeWalt circular saw (model DW364) was plugged to exposed employees to electrical hazards.  The extension cord's sheathing was separated and/or pulled from parts it was designed to protect near the cord's plug.     c. Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 14, 2015, the electrical pull box providing power and electrical controls to the  the Raimann ripsaw was damaged.  The durability of parts designed to enclose and protect electrical equipment in the panel exposed employees to electrical hazards in that the cover was damaged and would not remain closed.

29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses.    a. Care Caskets Int'l, LLC - Warton, Ohio: On or about October 14, 2015, the employer did not ensure that employees who operated woodworking equipment such as, but not limited to, the rip saw and planer wore eye protection incorporated in prescription lenses, or wore eye protection over their prescription glasses.  Employees were exposed to eye injury from flying wood debris.    Care Caskets Int'l, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.133(a)(1), which was contained in OSHA inspection number 803202, citation number 1, item number 1 and was affirmed as a final order on April 30, 2013, with respect to a workplace located at 200 E. Wyandot St. in Wharton, Ohio.

Care Caskets Int'l, LLC - Wharton, Ohio: On or about September 15, 2015, the employer did not ensure that the energy isolating device that is needed to control energy to the Timesaver Finishing Planer was operated so as to isolate energy while employees serviced the machine.  The planer was not locked out during maintenance activities such as, but not limited to, machine and blade cleaning.     b. Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 13, 2015, the employer did not ensure that the energy isolating device that is needed to control energy to the Raimann Rip Saw was operated so as to isolate energy while employees serviced the machine.  The saw was not locked out during cleaning, set-up, and blade changing activities.    c. Care Caskets Int'l, LLC - Wharton, Ohio: On or about August 28, 2015, the employer did not ensure that energy isolating device(s) that is needed to control energy to the Bridgewood Moulder machine (model CM-555) was operated so as to isolate energy while employees serviced the machine.  The machine was not locked out during repairs and set-up.    Care Caskets Int'l, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 803202, citation number 1, item number 2 and was affirmed as a final order on April 30, 2013, with respect to a workplace located at 200 E.
