Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 14, 2015, the flexible power extension cord to which the DeWalt circular saw (model DW364) was plugged to exposed employees to electrical hazards.  The extension cord's sheathing was separated and/or pulled from parts it was designed to protect near the cord's plug.     c. Care Caskets Int'l, LLC - Wharton, Ohio: On or about October 14, 2015, the electrical pull box providing power and electrical controls to the  the Raimann ripsaw was damaged.  The durability of parts designed to enclose and protect electrical equipment in the panel exposed employees to electrical hazards in that the cover was damaged and would not remain closed.
