(a)  Perez Construction, at 2200 Apres Ski Way, Steamboat Springs, CO 80487: On and before October 15, 2015, the exposing employer failed to ensure that employees performing work on a second story balcony were protected from falling through the use of adequate fall protective measures.  The condition exposed employees to fall hazards of approximately 20 feet as they worked installing decking/floors on a second floor balcony.
