OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: (a) On or about 10/19/2015 - Maintenance Shop, Hyster 60 Forklift #6, Model #H60XM, Serial #H177B34097Z, an employee was injured when he placed his finger into a hole in the area for a pin for a tilt cylinder during the process of replacement of the mast.
