29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 05/20/2016 - Mill Room; Stihl Holtec Cross Cut Saw; Serial #93160; Guard on the end of the gliding guide of the cutting bar facing the east side of the mill room was damaged. The barrier guard on the left side was broken and there was a gap in the guard enclosure that was 8" wide by 36" long exposing employees to the point of operation of the chain drive and flying chips when the saw was cutting packs of wood. b) On or about 05/20/2016 - Behind Mill Room; West Salem Machinery Company Horizontal Hog; Serial #2838RH; Sides of the shaker screen for the hog had in-running pinch points that were not guarded.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: a) On or about 05/20/2016: Hogger; Wood blocks, saw dust and metal poles used to rake the blocks away from the shaker screen area had been allowed to accumulate on the concrete pad under and around the hogger, hopper and shaker screen creating the potential for trip and fall hazards. b) On or about 05/20/2016: Platform on south end of Millroom, adjacent to the Hogger; A pallet, with pipes, wood dust and other debris was laying on the edge of the platform creating a potential for trip and fall hazards.
