29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    a). Granite Shop Building - On or about October 27, 2015, an employee was not protected from overhead hazards such as, but not limited to electrically powered tools when working underneath a elevated personnel platform, thus exposing employees to struck-by hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.    a). Granite Shop Building - On or about and times prior to October 28, 2015, a Terex Handler (model: SS636) not designed to support personnel platforms was used to support such a platform, thus exposing employees to a fall hazard of up to 16.5 feet onto earthen or concrete ground.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a). Granite Shop Building - On or about October 28, 2015, employees were installing metal decking on the roof without a fall protection system in place, thus exposing employees to a fall hazard of approximately 16.5 feet onto concrete or earthen ground.
