29 CFR 1910.23(a)(8): Every floor hole into which persons could accidently walk were not guarded: a) City of Philadelphia jobsite/2nd Floor transfer station - A section of the cat walk located above the refuse pit that was used by employees to perform service activities was missing, exposing employees to fall hazards on or about October 28, 2015.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by some type of fall protection system on all open sides: a) City of Philadelphia jobsite/2nd Floor transfer station - The employees were located near the edge of the refuse pit approximately 55 feet above the first floor without the use of some type of fall protection system thus exposing the employees to a fall hazard. This condition was observed on or about October 28, 2015
