29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Processing Work Area: On or about July 16, 2015, an annual inspection of the energy control procedures was not conducted, exposing employees to potential injury.  Abatement Note:   Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.1910.147(c)(6)(i)(D)   Where tagout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.1910.147(c)(6)(ii)   The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a) Processing Work Area, Batch Room: On or about November 4, 2015, employees were cleaning food processing machinery using AFCO 5288 McShine, containing injurious corrosive materials, without immediate access to suitable facilities for quick drenching or flushing of the eyes and body.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   a) 176 N. Commerce Way Bethlehem, PA:  On or about July 16, 2015, a worker suffered a finger-tip amputation, but the employer did not report the employees amputation to OSHA within 24 hours.
