29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof: a) For employees working at 13620 Oneco-Myakka City Road located in Bradenton as observed on or about 14 October, 2015. Employees were working in a three story wood frame new construction apartment complex without a fire extinguisher located in the building. The employees had a gasoline can and a gasoline powered compressor located in the middle of the third floor of the structure. Employees were exposed to a fire hazard while working on the roof of the building.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) For employees working at 13620 Oneco-Myakka City Road located in Bradenton as observed on or about 14 October, 2015. Employees were installing roof sheathing without the use of any fall protection system. Employees were exposed to a fall height of approximately 32 feet.
