29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about November 2, 2015, the Aerator was not guarded at the following locations: a) The clamping mechanisms where the large stacks of paper are clamped in place before being aerated, b) The control pedestal which was located inside the envelope of the laser curtain exposing the feet to entrapment by the pivoting aerator.
