29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees performed servicing tasks such as but not limited to clearing jammed paper from the Pemco Körber Paperlink Ream Wrapper Model 31R (Serial No. P-3635) without utilizing the employer established lockout tagout procedure, exposing employees to caught-in, struck-by, and electrical hazards associated with the unexpected release of energy, on or about August 7, 2018.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls was acquired by employees:  a) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees performed servicing tasks such as but not limited to clearing jammed paper from the Pemco Körber Paperlink Ream Wrapper Model 31R (Serial No. P-3635) without being trained to isolate the machine from the electrical and pneumatic energy sources, affix lockout devices to the isolation devices, and verify the isolation and de-energization of the machine, on or about August 7, 2018. 29 CFR 1910.212(a)(3)(ii): The points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:  a) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees created bales of waste cardboard with a damaged Wastequip Vertical Baler Model 7230 (Serial No. LB 10026) whose loading chamber access gate was unable to be fully closed thereby exposing employees to contact with the ram (point of operation) during each compaction cycle, on or about August 7, 2018.  OR IN THE ALTERNATIVE:  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees operated a Wastequip Model 7230 Vertical Baler (Serial No. LB 10026) with a modified loading chamber access gate interlock system, exposing employees to caught-in and crushed-by hazards, on or about August 7, 2018.  b) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees operated a Wastequip Model 7230 Vertical Baler (Serial No. LB 10026) with a loading chamber access gate with a damaged interlock system that did not prevent the ram from cycling while the gate was open, exposing the employees to caught-in and crushed-by hazards, on or about August 7, 2018.  c) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees operated a Wastequip Model 7230 Vertical Baler (Serial No. LB 10026) with a damaged bale chamber door (bent) that could fail during operation exposing the employees to struck-by hazards, on or about August 7, 2018.  In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Area, 830 Crowe Road, East Stroudsburg, PA - Employees operated the Pemco Körber Paperlink Ream Wrapper Model 31R (Serial No. P-3635) with the plexiglass guard doors (i.e guards) raised exposing employees to ingoing nip point and rotating part hazards when manufacturing paper reams, on or about August 7, 2018.  In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.  Northwoods Paper Converting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1130512, citation number 1, item number 2 and was affirmed as a final order on May 4, 2016, with respect to a workplace located at 830 Crowe Road, East Stroudsburg, PA.
