29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about November 5, 2015, two employees using Makita circular handsaws to cut framing pieces were not wearing safety glasses, thereby exposing themselves to being hit or struck-by flying debris.   b. On or about November 5, 2015, three employees using Hitachi NR83A2 pneumatic nail guns to conduct framing operations were not wearing safety glasses, thereby exposing themselves to being hit by nails or flying debris.

On or about November 5, 2015, in a 4 x 4 Skyjack boom lift, an employee performing framing operations was not protected from a 10-foot fall hazard by use of a personal fall arrest system.

On or about November 5, 2015, on the top plate of a structure, 15 employees performing framing operations were not protected from a 10-foot fall hazard by use of a guardrail system.
