29 CFR 1903: PART 1903 - INSPECTIONS, CITATIONS AND PROPOSED PENALTIES  Sec.  29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section:  a.  The employer did not furnish the abatement required for citations issued within 10 calendar days for Citation 1 Item 1, nor did they respond to the progress report letter sent November 25, 2016.  b.  The employer did not furnish the abatement required for citations issued within 10 calendar days for Citation 1 Item 2, nor did they respond to the progress report letter sent November 25, 2016.  c. 29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a. Site - Employees were not protected from slip, trip, fall, and puncture hazards when debris was allowed to accumulate around the jobsite which included but was not limited to shingles with protruding nails.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Site - Employees performing roofing work were exposed to falls of approximately 28 feet when the aluminum extension ladder being used to gain access to and from the roof was only extended approximately 6 inches above the eave.
