29 CFR 1910.23(a)(4): Every skylight floor opening and hole was not guarded by a standard skylight screen or a fixed standard railing on all exposed sides. On November 11, 2015 an employee was on the roof of the building and fell through a skylight that did not have a skylight screen or a fixed railing. The employee fell approximately 17.4 feet onto a mezzanine inside the building. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
