29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):       On or about November 1, 2017, an employee opened the lid to the hopper to perform an inspection related to a malfunction of material flow and was not required to perform the following actions to isolate hazardous energy sources (lockout):     a) turn off and shut down the machine,   b) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),    c) affix lockout or tagout devices to each energy isolating device,   d) verify that isolation and de-energization of the machines or equipment have been accomplished.     Employees were exposed to amputation and laceration hazards when opening the hopper door for cleaning or checking the levels of material.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about November 1, 2017 to present, material handlers and maintenance employees were not provided with an adequate, documented procedure for cleaning activities or maintenance work performed inside the hopper when the cover was removed. Employees were exposed to hazards likely to cause amputation.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about November 1, 2017 through February 14, 2018, the procedure that was in place for maintaining, inspecting grinders did not address a specific method to perform verification of isolation of hydraulic and electrical energy including rendering safe all stored energy prior to starting work on the machine. The procedure also did not contain a specific statement for intended use during inspection of the hopper or other components, including the air lock (rotary valve).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
