29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a: On 11/9/15 the floor under and around rail extruder line # 2 was not dry and clean in that oil was allowed to accumulated in that area, resulting in an employee slipping on the oil and contacting an unguarded moving machine part.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
