29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) Long View RV Job Site - On or about 11-18-15 - Employees were observed not properly using personal fall arrest systems and warning line systems on this job site. Employees did not buckle harness systems to their legs and warning lines were allowed to sag, while employees were observed working outside of the warning line area without being tied off.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Long View RV Job Site - On or about 11-18-15- Employees were exposed to fall hazards of 12 feet above the ground while installing solar panels on the metal roof of the building with no fall protection systems being used. The Green Earth Energy Photovoltaic Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1081126, citation number 2, item number 1 and was affirmed as a final order on 9/10/15, with respect to a workplace located at Crowley Dealership, 1451 Farmington Avenue, Bristol, CT.
