Main St., East Longmeadow, MA: On or about October 3, 2016, an employee was installing part of a solar panel system on the roof of this building more than 13 feet above the ground below, and his snaphook was engaged to his horizontal lifeline which was wrapped around a gas line on the roof, which was secured to the roof via a C clamp to a block of wood, which is not intended to support any imposed load.

Main St., East Longmeadow, MA: On or about October 3, 2016, an employee was installing part of a solar panel system on the roof of this building more than 13 feet above the ground below, and his fall protection ropes were rigged so that if he fell, he would contact the ground he was working above as the fall was 13 feet, and he had more than 20 feet of lifeline to deploy before his fall was arrested.
