29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    Worksite: The employer did not ensure that the employees required to perform roofing work on the roof of a commercial building were protected from falling through unguarded skylights by the use of appropriate protective systems/devices.       On or about 06/08/2018, one of employees performing roofing work on the above roof fell approximately 12 feet 5 inches to lower level through one of unguarded skylights and sustained minor injuries from the incident.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Worksite: The employer did not ensure that the employees working on the low sloped roof at a height above 6 feet were protected from falling by guardrails, safety nets, personal fall arrest systems or a monitoring system.      Green Earth Energy Photovoltaic Corporation was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(1) which was contained in OSHA Inspection Number 1106885, Citation Number 2, Item Number 1, issued on 12/11/2015 with respect to a workplace located at 27 Lawnacre Road, Windsor Locks, CT and became a final order on 12/22/2015.
