29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. Employees performing roofing work are not protected from eye injuries while using pneumatic nail guns.      The employer routinely fails to assure effective protection against eye injuries as required by the standard, as demonstrated on November 23, 2015, when employees were working on a residential roof at 11895 Swooping Willow Road, Jacksonville, Florida 32210 without eye protection while using pneumatic nail guns and were exposed to eye injuries from flying debris or nails.      Jasper Contractors, Inc. previously committed a serious violation of this standard on September 23, 2014, with respect to a workplace at 355 Silent Brook Trial, Jacksonville Florida which became a final order on February 18, 2016 (Inspection No. 976501).       Jasper Contractors, Inc. previously committed a repeat-serious violation of this standard on August 28, 2014, with respect to a workplace at 11760 Heather Grove Lane, Jacksonville Florida which became a final order on February 18, 2016 (Inspection No. 992409).        Jasper Contractors, Inc. previously committed a serious violation of this standard on September 11, 2012, with respect to a workplace at 2927 Newbury Court, Highlands Ranch, CO, which became a final order on November 28, 2012 (Inspection No. 626158).        Jasper Contractors, Inc. previously committed a serious violation of this standard on July 25, 2012, with respect to a workplace at, 10775 Glengate Loop, Littleton, CO, which became a final order on November 28, 2012 (Inspection No. 546338).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. Employees performing re-roofing work from a 7:12 pitch roof are not being protected from 7 feet 10 inches, and up to 16 feet, fall hazards by a fall protection system.    The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on November 23, 2015, when employees were installing shingles on the roof of a one-story residence at 11895 Swooping Willow Road, Jacksonville, Florida 32210 without fall protections and were exposed to fall hazards of 7 feet 10 inches, and up to 16 feet, above the surrounding ground.    Jasper Contractors, Inc. previously committed a repeat-serious violation of this standard on December 2, 2014, with respect to a workplace at 3301 Shelley Drive, Green Cove Springs, Florida which became a final order on February 18, 2016 (Inspection No. 1011179).        Jasper Contractors, Inc. previously committed a willful violation of this standard on March 28, 2014, with respect to a workplace at 12832 Kelsey Island Drive, Jacksonville Florida which became a final order on February 18, 2016 (Inspection No. 965323).
