29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: a) 75 Grant Avenue, Fords, NJ 08863. Employees were operating pneumatic nails guns when installing new roofing materials while not wearing any eye and or face protection exposing them to eye and face injuries from flying debris. Condition occurred on or about 11/18/15.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: a) 75 Grant Avenue, Fords, NJ 08863. Employees were accessing the roof using the fly section of a Werner 28 foot aluminum ladder. The ladder was not being used as intended in that it was missing both Safety Shoes aka foot pads exposing the employees to fall hazards due to possible displacement. Condition occurred on or about 11/18/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 75 Grant Avenue, Fords, NJ 08863. Employees were installing new roofing materials when working on and from the roof of a two story home with no fall protection systems in place exposing them to fall hazards. Condition occurred on or about 11/18/15. A & B Builders Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1082952, citation number 1, item number 4 and was affirmed as a final order on October 14, 2015, with respect to a workplace located at 58 Liverpool Way, Block 404.28 - Lot 6, Medford NJ.
