29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as opening process equipment or piping: (a) Unit 27 Alkylation: On or about November 29, 2015, the employer failed to develop and implement safe work practices governing the management of flammable liquids (LPG material) diverted to the oily water sewer from a process vessel during the preparation for maintenance of the 27-D-200 Caustic Wash Drum and the 27-S-201 Coalescer Drum.

29 CFR 1910.119(e)(3)(i): The process hazard analysis did not adequately address certain hazards of the process: (a) Unit 27 Alkylation: On or about November 29, 2015, the employer's PHA for the Unit 27 Alkylation unit had not adequately addressed the hazards associated with flammable (LPG) materials potentially being drained into the oily water sewer in preparation for maintenance of the 27-D-201 Caustic Wash Drum and 27-S-201 Coalescer Drum." Pursuant to 29 CFR 1903.19, the employer must submit abatement certification and documentation, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
