29 CFR 1910.119(j)(2): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices (RAGAGEP): a) Delaware City Refining Company, 4550 Wrangle Hill Road, Delaware City, Delaware; Fluid Coker Reactor - On or about April 9, 2019, the employer failed to follow RAGAGEP for the postweld heat treatment of a replaced section of the Coker Shell, exposing employees to a fire hazard. Abatement Note: Both the American Petroleum Institute's API 510, Pressure Vessel Inspection Code: In-Service Inspection, Rating, Repair, and Alteration and American Society of Mechanical Engineers' PCC-2, Repair of Pressure Equipment and Piping, Article 2.14, provide guidance for post weld heat treatment as does the National Fire Protection Association's NFPA® 51B Standard for Fire Prevention During Welding, Cutting, and Other Hot Work.

The Delaware City Refining Company, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.119(f)(4), which was contained in OSHA inspection number 1108618, citation number 1, item number 1, and was affirmed as a final order on 12/21/2016, with respect to a workplace located at 4550 Wrangle Hill Road, Delaware City, DE 19706.
