29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that are outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation:  a) Crude Unit Gasoline Column C-1 - On or about February 3, 2019, the employer failed to insulate and heat trace the entire Gasoline Column Water Draw-Off Pot (D-12) Drain Line originating at the bottom of D-12 and pumped through the line by Pump P-20 to the Sour Water Treatment Area, resulting in this line rupturing during a cold snap then releasing flammable vapors that ignited into a fire spreading throughout the Crude Unit.
