29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    On or about December 1, 2015, at the job site employees were observed working on a second and third levels a three-tier scaffolds which did not have the working levels fully planked between the uprights.  The employees were exposed to fall hazards.    I & M Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1065331, citation number 1, item number 1, and was affirmed as a final order on October 2, 2015 with a final abatement date of May 21, 2015, with respect to a workplace hazard at 4611 W. 6th Ave., Stillwater, OK 74074.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     On or about December 1, 2015, at the job site employees were observed working from second and third levels of 3-tier scaffolds.  The scaffolds did not have guard rails provided and the employees were not wearing personal fall arrest harness.  Employees were exposed to fall hazards.     I & M Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1065331, citation number 1, item number 3, and was affirmed as a final order on October 2, 2015 with a final abatement date of May 21, 2015, with respect to a workplace hazard at 4611 W. 6th Ave., Stillwater, OK 74074.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about December 1, 2015, at the job site employees were observed not wearing hard hat while working on lower levels in the areas where there was a possible of head injury caused by falling objects from other employees working above.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:    On or about December 1, 2015, at the job site employees were observed working from second level and third level of a 3-tier scaffold which the scaffolds poles, legs, posts, frames and uprights were provided with diagonal braces in the front only not in the back.  Employees were exposed to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about December 1, 2015, at the job site employees were observed scaling on the scaffolding frames as a means to access the scaffolds, no ladder was provided.  Employees were exposed fall hazards.
