29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    On or about January 27, 2016, at hotel renovation site located Tulsa, OK, employees were allowed to work on a scaffold that was not fully planked.  The employees were exposed to fall hazards of more than 10 feet.

29 CFR 1926.451(f)(7):  Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  Such activities were not performed only by experienced and trained employees selected for such work by the competent person:       On or about January 27, 2016, at hotel renovation site located in Tulsa, OK, the employer did not inspect a scaffold prior to allowing employees to access and commence work on the scaffold, exposing the employees to the hazards of falls.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level:                                                                                                                                           On or about January 27, 2016, at hotel renovation site located Tulsa, OK, employees were observed working on a scaffold that did not have guardrails around all open ends, exposing the employees to the hazards of falls.
