29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:     On or about August 30, 2016, at a new structure construction site located at 12624 S. Memorial in Tulsa, OK, employees were allowed to work on a scaffold that was not fully planked, exposing the employees to fall hazards.       I.M. Stucco L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1065331, citation number 1, item number 1, and was affirmed as a final order on October 2, 2015, with a final abatement date of May 21, 2015, with respect to a workplace hazard at 4611 W. 6th Ave. Stillwater, OK 74074.     I.M. Stucco L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1109669, citation number 1, item number 1, and was affirmed as a final order on March 11 2016, with a final abatement date of December 15, 2015, with respect to a workplace hazard at 1331 E. 71st Street Tulsa, OK 74136.     I.M. Stucco L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1121536, citation number 1, item number 1, and was affirmed as a final order on May 6, 2016, with a final abatement date of February 3, 2016, with respect to a workplace hazard at 44 South 193rd E. Ave. Tulsa, OK 74108.

Memorial in Tulsa, Ok, scaffolding and components were not inspected before each shift, exposing employees to the hazard of falls.    I.M. Stucco L.L.C.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(f)(7), which was contained in OSHA inspection number 1121536, citation number 1, item number 2, and was affirmed as a final order on May 6, 2016, with a final abatement date of February 3, 2016, with respect to a workplace hazard at 44 South 193rd E. Memorial in Tulsa, OK, employees were observed working on a scaffold that did not have guardrails around all open ends, exposing the employees to the hazard of falls.        I.M.
