29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At the jobsite located at 201 E. Kansas, McPherson, KS, employees engaged in roofing activities from a 7:12 roof were exposed to fall hazards of approximately 12 feet from the eave of the building to the ground below in that they were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 201 E. Kansas, McPherson, KS, employees engaged in roofing activities from a 7:12 roof were exposed to fall hazards of approximately 12 feet from the eave of the building to the ground below in that  the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
