29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. Buildings 3301, Southeast exit; on or about 01 December 2015 and times prior; the employer did not ensure that emergency exits lights were in proper working order. The emergency exit lights located above the doorway leading to the southeast exit were not in working order. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving. Workbench area; on or about 01 December 2015 and times prior thereto; the employer did not ensure that machines were secure. Two instances were noted: a. A Wilton 8 inch bench grinder, serial number unknown, was not securely mounted to prevent movement. b. A Wilton 6 inch bench grinder, serial number unknown, was not securely mounted to prevent movement. This condition exposed employees to a struck by hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Mechanical room, on or about 01 December 2015 and times prior thereto; the employer did not ensure that the electrical panels were effectively closed. A 208 volt energized electrical panel board labeled C had an open blank on breaker slot 26. This condition exposed employees to an electrical shock, electrical burn hazards and thermal hazards.
