29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation: (a) OSHA 300 Forms for the calendar year 2013-2015 - Deficiencies were noted: - In year 2013, several injuries had only the employee's name and location of the accident, and one incident, had only the employee's name. - In 2015 three entries had only the employees name and job title and two other were missing the location of the incident. These omissions were observed on OSHA 300 forms on or about 3-14-2016. Abatement certification required.
