29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about December 10, 2015, the employer failed to provide fall protection equipment including but not limited to personal fall arrest systems and approved anchor points for employees walking on the metal frame, located in the Timken Inspection area. The employee was observed working from the height greater than 6 feet to maneuver material onto a fork truck.
