29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Drill site: On or about December 8, 2015, and at times prior thereto, employees were working on Nomac Drilling Rig # 54. A water pump box located behind the trailers had a power strip/tap (for indoor use only) supplying power to equipment. This condition exposed employees to electrical shock and fire hazards.

Behind the crew trailers three extension cords were plugged into one another (daisy-chained). This condition can exposed employees to electrical shock hazards.
