29 CFR 1910.147(d): The established procedure for the application of energy control (the lock out or tag out procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Employees conducting machine servicing, such as cleaning the metal guide on a Elliot Case Sealer Machine, Model Number #80-14, Serial Number 90-6-1, were exposed to amputation hazards and energy isolating devices were not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources, and lock out or tag out devices were not affixed to each energy isolating device. The employer did not require that employees follow the sequence outlined in 29 CFR 1910.147.
