29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:    Packaging department, mezzanine, packaging line no. 15 bagger scales: on or about January 4, 2016 and dates thereafter, employees engaged in cleaning and/or sanitation tasks were exposed to fall-through hazards where a floor opening around the equipment was not guarded. 29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    Packaging department, mezzanine, packaging line no. 13 bagger scales: on or about January 4, 2016, and dates thereafter, employees engaged in cleaning and/or sanitation tasks on the equipment were exposed to falling through the opening where a mid rail was missing on the work platform. Employees were exposed to a fall hazard of up to approximately eight (8) feet onto the concrete floor below.    There is no abatement certification or documentation required for this item.

29 CFR 1910.305(g)(1)(iii): Flexible cords used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of 29 CFR 1910.305 were not equipped with an attachment plug and energized from an approved receptacle outlet:    Packaging department, packaging lines 14 to 23: on or about January 4, 2016, and dates thereafter, the employer failed to energize flexible cords through approved receptacle outlets. Up to four flexible cords were permanently wired (hard wired) into metal junction boxes anchored to vertical support beams. Employees were exposed to electric shock and/or burns when connecting equipment power cords to such unapproved receptacle outlets.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.     Packaging department, packaging lines 14 to 23: on or about January 4, 2016, and dates thereafter, employees walking in aisles between equipment were exposed to trip and fall hazards where flexible electrical power cords were laying on the floor or hanging above the ground level to connect to other flexible cord receptacles that extended from the electrical boxes affixed to vertical support beams.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    The Snak-King Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 895442, citation number 01, item number 001 and was affirmed as a final order on June 17, 2013, with respect to a workplace located at 3133 Industrial Dr., Freeport, IL 61032.

29 CFR 1910.178(l)(4)(ii)(D): Refresher training in relevant topics was not provided to the operator when the operator was assigned to drive a different type of truck:    Warehouse, Toyota electric sit-down counterbalanced powered industrial vehicles - on or about January 4, 2016, employees were exposed to hazards associated with unsafe operating of forklifts during material handling work tasks. Adequate refresher training with practical exercises and evaluation in safe operation of new forklifts were not conducted when operators were assigned to different type of forklifts equipped with substantially different operating controls.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
