29 CFR 1910.101(b): Section 3.4.4, Compressed Gas Association Pamphlet P-1-1965, as incorporated by reference in Sec. 1910.6: Compressed gas cylinder(s) not in use were not properly supported to prevent them from being knocked over:     A.  The employer failed to ensure that one (1) K size (244CuFt) compressed gas cylinder, located in the Band Mill, containing oxygen was properly supported.    B.  The employer failed to ensure that one (1) K size (244CuFt) compressed gas cylinder, located in the Big Mill, containing argon was properly supported.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer did not establish an energy control program for employees who were performing serving operations on equipment such as, but not limited to, Wood-Mizer sawmill.  Servicing operations included, but not limited to, blade changing.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about 12/29/2015, employees were exposed to machine hazards associated with moving parts and in-going nip points during servicing and blade changing operations, of AWMV Wood-Mizer, located in the Band Mill area.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6), were not implemented to protect employees from machine servicing hazards.

No architectural appurtenance or other equipment may be located in the working space required by paragraph (g)(1)(i) of this section.    The outdoor electrical feeder that is located on the back of the Big Mill building, was damaged and was not installed in a suitable enclosure or protected from accidental contact.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:       Flexible cords and cables were used as permanent wiring when:  A.       An orange flexible (extension) cord ran from an outlet located on a building next to the Band Mill to an area under the mill machines.  B.       A yellow flexible (extension) cord was ran from the orange extension cord to provide power to a multi tap outlet extension cord.  C.       A yellow multi-tap outlet extension cord was ran from the yellow extension cord to provide power to a circular saw and an orange extension cord.  D.      An orange extension cord was ran from a yellow multi-tap outlet extension cord and to the other side of the mill machine to provide power to a portable electrically powered hand tool.
