29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized. If such controls fail to reduce sound levels within the levels of Table G-16, personal protective equipment were not provided and used to reduce sound levels within the levels of the table: When employees working in the Band Mill, Saw Mill and operating the chipper, were subjected to sound in excess of the action level for noise, the employer had not utilized administrative controls, engineering controls or personal protective equipment to reduce sound levels within the levels of Table G-16. An employee was exposed to continuous and intermittent noise at 169.6% of the action level or an equivalent sound level of approximately 93.8 dBA during the 464 minute sample period. An employee was exposed to continuous and intermittent noise at 101.2% of the action level or an equivalent sound level of approximately 90.1 dBA during the 447 minute sample period. An employee was exposed to continuous and intermittent noise at 162.6% of the action level or an equivalent sound level of approximately 93.5 dBA during the 437 minute sample period. An employee was exposed to continuous and intermittent noise at 133.3% of the action level or an equivalent sound level of approximately 92.1 dBA during the 445 minute sample period. An employee was exposed to continuous and intermittent noise at 403.5% of the action level or an equivalent sound level of approximately 100.1 dBA during the 440 minute sample period.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: On or about December 29, 2015, the employer did not record the following workplace injuries and illness on the OSHA 300 log for the calendar year 2015: On or about November 11, 2015, a recordable injury occurred in the Bridge Mat area when an employee was walking in between an assembly line and a bridge mat and roller struck the side of the employee's stomach and lower back. The employee was taken to the hospital for treatment and was away from work until January 28, 2016.
