29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section: The employer did not ensure that written hazardous energy control procedures were developed and implemented that addressed the scope & purpose of the procedure, specific steps for shutting down and isolating energy sources, specific steps for placing and removing lockout/tagout devices, and specific steps for verifying the de-energization of lumber processing machinery,which includes but is not limited to, the re-saw, head saw, and chipper.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about May 2nd, 2017, employees were exposed to machine hazards associated with moving parts and nip points during servicing of the re-saw machine located at the re-saw building. The employer failed to implement energy control procedures, as the machine was not shut down or turned off to clear jammed plank from the machine [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
