29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage: A. On or about January 10, 2016, at Towel Folding Machine #1: A mechanic removed the rear safeguard to the machine to separate overlapping belts on the energized equipment. The employee sustained a serious laceration injury to the finger and thumb on the left hand, requiring hospitalization.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: A On or about January 14, 2016, Towel Folding Area: At Chicago Dryer Company Towel Folding Machine #2, when a maintenance employee attempted to shut down the equipment at the 480V switch, the switch cover opened. The switch was located near the front of the equipment and subject to damage by rolling carts. The uncovered switch exposed employees to electric shock hazards.
