29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition. Production Area; On or about 10/23/18, the employer had failed to keep surfaces free of the accumulation of lint created from the commercial laundry operation. Lint accumulated on beams and other surfaces resulted in a fire that ignited and burned across those beams.

29 CFR 1910.156(c)(1): The employer did not provide training and education commensurate with those duties and functions that fire brigade members are expected to perform: Production Area; On or about 10/23/18, the management and maintenance employees used 18-25 dry chemical fire extinguishers to fight a spreading lint fire in the production area. Their level of training was fire extinguisher training, last received in January 2016.

Production Area; On or about 10/23/18, employees used compressed air to clean lint that accumulated on surfaces using a compressor running at 100 p.s.i. without the use of a pressure reducing tip on the end of the air gun.
