29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: a) 7132 Ruppsville Road, Allentown, Roof - on or about January 6, 2016, employees were applying sheet metal to the roofing curbing using a portable electric drill without any means of eye protection.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) 7132 Ruppsville Road, Allentown, Roof- On or about January 6, 2016, employees were conducting commercial roofing work on a low slope roof, working between approximately 16 to 40 feet above grade, was not protected from falling by the combination of a warning line system and safety monitor.
