29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) 305 S Richland Ave - The employer did not protect employees who were removing and installing sheathing from falling through holes by ensuring the use of personal fall arrest systems, covers or guardrails, thereby, exposing employees to falls 19 feet to the floor below on or before January 26, 2017.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) 305 S Richland Ave - Employees conducting work on a commercial roof with a 4/12 pitch and working between heights of 11 feet and 19 feet were not protected from falling by personal fall arrest system, thereby, exposing employees to fall hazards of 19 feet to the ground below on or before January 25, 2017    Ream Roofing was previously cited for a violation of this occupational safety and health standard 1926.501(b)(10) which was contained in OSHA inspection number 1116725 citation number 1 item number 2 and was affirmed as a final order on March 9, 2016 with respect to a workplace located at 7132 Ruppsville Road, Allentown, Pa.      Ream Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number903839, citation number 1, item number 5 and was affirmed as a final order on August 22, 2013, with respect to a workplace located at 472 East Locust Street Ste 4 Dallastown PA 17313.    Ream Roofing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 308479609, Citation Number 2, Item Number 1 and was affirmed as a final order on December 21, 2006, with respect to a workplace located at 31 South Broad Street, York, PA 17405.      Ream Roofing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 308479229, Citation Number 1, Item Number 1 and was affirmed as a final order on October 24, 2006, with respect to a workplace located at 550 Homestead Road, Hershey, PA 17033.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
