29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: a.) Corporate Glass, Inc., 2415 Pittsburgh Ave., Erie, Pa, 16502, Fabrication Shop area, On or about January 13th, 2016, and times prior thereto: A Ryobi bench grinder (serial # W020801371) work rest was greater than 1/8 of an inch from the abrasive wheel on the left side of the grinder. The Ryobi bench grinder was missing the work rest on the left side of the grinder which created an opening of approximately 1/2 inch from the abrasive wheel.

Corporate Glass, Inc., 2415 Pittsburgh Ave., Erie, Pa, 16502, Dewalt miter saw area of the Fabrication Shop, On or about January 13th, 2016, and times prior thereto: Employees utilized compressed air to clean off metal/wood shavings in the Dewalt Miter saw area. Measurements of compressed air next to the Dewalt Miter saw was measured with an air regulator gauge and was greater than 60 p.s.i.

Some employees did not receive training on the new labeling elements and safety data sheet format by the December 1, 2013 deadline noted in 29 CFR 1910.1200 (j)(1) of the Hazard Communication 2012 Standard.

29 CFR 1910.106(e)(9)(i): Maintenance and operating practices did not control leakage and prevent accidental escape of flammable liquids: a.) Corporate Glass, Inc., 2415 Pittsburgh Ave., Erie, Pa, 16502, Glass cutting and Fabrication shop areas, On or about January 13th, 2016, and times prior thereto: Employees were potentially exposed to a fire hazard during dispensing operations of flammable liquids. Employees periodically dispensed flammable liquids including, but not limited to, Denatured Alcohol (Sunnyside brand) and Methyl Ethyl Ketone (Sunnyside brand) onto work rags in order to perform operations including, but not limited to, glass cleaning. Employees were pouring the flammable liquids onto work rags which potentially created a spill hazard.
