The lockout procedures did not provide sequential steps to bring the machinery to a zero energy state.  In addition, the lockout procedures were missing information including, but not limited to, the intended use of the procedure; sources and magnitudes of energy; notification of affected employees prior to and following the lockout of equipment; specific procedural steps for shutting down, isolating, blocking and securing equipment; specific procedural steps for placement, specific techniques for bleeding, blocking and locking lines, removal and transfer of lockout devices or tagout devices and the responsibility for them; specific requirements for testing equipment determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures; and steps to release the lockout.
