The cover shall be securely held in place to prevent tools or materials from falling through:  (a) Pipe Plant - On or about December 10, 2015 the Hawkeye precast concrete pipe forms had floor openings from 1 to 2 feet in width thereby exposing employees to the hazard of falling 7 feet to the concrete basement below.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards; Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  (a) Pipe Plant - On or about December 10, 2015 an employee using a shovel to remove jammed concrete inside the concrete hopper bin was exposed to a 10 foot fall hazard. The employee was straddled over a conveyor belt that was 4 feet below him. He had his left foot on top of the fixed ladder and his right foot on top of the spinning shaft guard plate without the use of any fall protection or guard rail system.
