29 CFR 1910.23(a)(3): The employer did not ensure that every hatchway and chute floor opening was guarded by either a hinged floor opening cover of standard strength and construction equipped with standard railings or permanently attached thereto so as to leave only one exposed side, or by a removable railing with toeboard on not more than two sides of the opening and fixed standard railings with toeboards on all other exposed sides:   (a)  Pipe Plant - On or about August 4, 2016 the open pit for the hopper bucket that feeds the Hawkeye machine was not protected by a guard rail system to prevent employees from falling into pit.

(a) Pipe Plant - On or about August 4, 2016 employer was using a rough terrain extending boom fork lift truck that had a 4 foot X 4 foot wooden skid pallet on the forks to lift employee 30 feet high to fix air leak. The wooden skid pallet was not provided with standard railings.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Pipe Plant - On or about August 4, 2016 the Maintenance Supervisor was fixing an air leak on the overhead gantry crane 30 feet high that was not locked out or tagged out to prevent the crane from accidently operating during the maintenance of the air leak.

The employer failed to get written approval of the attachment by the manufacture of the lift.
