29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At the job site, on or about January 21, 2016, an employee exposed to overhead hazards was not wearing a hard hat, exposing employee to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) At the job site, in back of the building, on or about January 21, 2016, an employee was exposed to fall hazard while using a 2 X 24 metal platform as a walkway, exposing him up to a 10-foot fall hazard.  b) At the job site, on or about January 21, 2016, two employees were working from the roof installing roof sheathing onto the roof deck without any means of fall protection, exposing employees up to 17-foot fall hazard.
