29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) At the job site, on or about July 08, 2016, several employees who were performing framing work on a new two-story single family residence were exposed to fall hazards up to approximately 9 feet onto the 2nd floor wooden deck because no fall protection was utilized.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) At the job site, on or about July 08, 2016, an employee was observed standing on the 1st step from the top of an 8-foot A-frame step ladder installing joist hangers onto the floor joists, resulting in a fall hazard.    b) At the job site, on or about July 08, 2016, an employee was observed standing on the top step of a 4-foot A-frame step ladder installing joist hangers onto the floor joists, resulting in a fall hazard.                                       c) At the job site, on 2nd floor, on or about July 08, 2016, two employees were observed using the top 2 steps of a 6-foot A-frame step ladder exiting the ceiling joists, resulting in a fall hazard.
