On or about January 25, 2016 and at times prior, an employee was exposed to electrical shock while operating a Corian heating form table. A hole opening on the junction box was not provided with a plug cover. The opening was beside the on/off switch of the Corian heating table.

On or about January 25, 2016 and at times prior, an employee was wearing a 3M Model 8200 particulate respirator while sanding Corian countertops. The employee was not provided with the information contained in Appendix D of 29 CFR 1910.134 prior to wearing the respirator.
