29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or are likely to be present, which would necessitate the use of personal protective equipment (PPE):   a) Facility, where the employer did not perform a hazard assessment to evaluate the need for protective equipment during tasks where products were used including:  1. Janitorial, where employees utilized corrosive chemicals including, but not limited to, Clorox Germicidal  Bleach.  2. Battery Room, where employees added distilled water to batteries, no face shield was provided.  3. Main Shop, where employees changed the propane cylinder on the Hyster Fortis 50 forklift.  4. Carpenter Shop, where employees sawed treated lumber.
