29 CFR 1910.141(a)(3)(i): All places of employment shall be kept clean to the extent that the nature of the work allows. (a) Locomotive Shop; There were metal shavings on the floor underneath the pipe threader and metal dust fragments underneath the miter saw that was used to cut metal pipes. This exposes employees to possible tripping hazards, laceration hazards and metal dust exposure hazards.

29 CFR 1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. (a) Storage Room attached to Locomotive Shop; An Amerex fire extinguisher was not mounted and sitting on the floor. The exposes employees to tripping hazards if the extinguisher were to fall over. (b) Locomotive Shop; An Amerex fire extinguisher was sitting on top of a flammable storage locker, not mounted. This exposes employees to possible struck by hazards if the extinguisher were to fall over and hit someone accessing the flammable storage locker.

29 CFR 1910.157(e)(2):Portable extinguishers or hose used in lieu thereof under paragraph (d)(3) of this section shall be visually inspected monthly. a) Warehouse and Old Locomotive Shop; None of the fire extinguishers located in employee work areas were visually inspected monthly, several Amerex fire extinguishers were located throughout the workspace. This exposes employees to possible smoke inhalation hazards if they try to extinguish a small fire and the fire extinguisher is inoperable. The U.S. Geological Survey was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1168135, citation number 3, item number 2 and was affirmed as a final order on November 7, 2016, with respect to a workplace located at Nordland, WA.

29 CFR 1910.1200(g)(8):The employer shall maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.) (a) Warehouse- Carpenter/ Woodworking Shop; The employer did not have safety data sheets for hazardous substance employees use, including but not limited to: Clorox bleach, Klean Strip Paint Thinner, Tacoma Screw High Solids Protective Coating, Valspar Paint, Save A Chain bar and chain lubricant, and Weld-on PVC. The U.S. Geological Survey was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1313397, citation number 1, item number 3C and was affirmed as a final order on July 23, 2018, with respect to a workplace located at Port Angeles, WA.
