Metco Industries, Inc.; press area - On or about 12/29/16, and at times prior thereto, the mode selector keys were left in the sizing presses. Operators and other unauthorized personnel had access to the keys.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. A.) Metco Industries, Inc.; 1241 Brusselles St, St. Mary's, PA - On or about 12/29/16, and at times prior thereto, the company did not report a work related amputation within 24 hours of it occurring. The amputation occurred on 12/29/15 at 10:00 PM. The amputation was not reported to OSHA until 1/28/16 at 1:00 PM.
